El principi actiu \u00e9s la diacetilmorfina, que actua sobre el sistema nervi\u00f3s central amb efectes depressors. Altres derivats de la morfina s\u00f3n els opiacis (per ex. metadona) i els opioides (per ex. tramadol).<\/p><p>Els efectes immediats s\u00f3n analg\u00e8sics, sedants, euforitzants i de plaer. Amb el pas del temps el que es busca \u00e9s alleujar el malestar que provoca l\u2019abs\u00e8ncia d\u2019hero\u00efna. Es produeixen alteracions de la personalitat, problemes de mem\u00f2ria, ansietat i depressi\u00f3, alteracions gastrointestinals, hep\u00e0tiques i renals.<\/p><p>El percentatge de persones que consumeixen hero\u00efna respecte a la poblaci\u00f3 general \u00e9s baix, aproximadament un 0,1 % per a les persones entre 15 i 64 anys. L\u2019edat mitjana d\u2019inici en el consum oscil\u00b7la entre els 14 i els 21 anys. Actualment hi ha un repunt del nombre de persones addictes, que es relaciona directament amb el canvi de perfil de l\u2019usuari en les noves generacions.<\/p><p>En els darrers anys la via d\u2019administraci\u00f3 ha canviat radicalment. Als anys 80 i 90 el consum principal era per via injectada. Actualment (associat principalment a l\u2019epid\u00e8mia de VIH\/sida i als estralls i morts que va produir entre aquesta poblaci\u00f3) s\u2019utilitza majorit\u00e0riament la via esnifada o b\u00e9 la fumada.<\/p><p>Molt sovint el consumidor d\u2019hero\u00efna ho \u00e9s tamb\u00e9 d\u2019altres subst\u00e0ncies (amfetamines, coca\u00efna, c\u00e0nnabis\u2026) i amb l\u2019hero\u00efna es busca sovint l\u2019efecte sedant que contrarresti el que li provoquen altres subst\u00e0ncies excitants, de manera que el perfil de l\u2019usuari ja no seria el d\u2019un hero\u00efn\u00f2man sin\u00f3 m\u00e9s aviat el d\u2019un politoxic\u00f2man.<\/p><p>\u00c9s una de les drogues m\u00e9s addictives que existeixen sigui quina sigui la via d\u2019administraci\u00f3; el seu consum genera r\u00e0pidament toler\u00e0ncia al cos,\u00a0 a les poques preses cal prendre\u2019n m\u00e9s quantitat per continuar notant els mateixos efectes. Aix\u00f2 la situa segurament com la droga amb m\u00e9s potencial d\u2019addicci\u00f3: una de cada quatre persones que la proven desenvolupa una addicci\u00f3.<\/p><p>Produeix a m\u00e9s s\u00edndrome d\u2019abstin\u00e8ncia quan s\u2019interromp el consum, cas en qu\u00e8 cal ser tractat m\u00e8dicament.<\/p><p>Els canvis estructurals i funcionals que produeix al cervell solen persistir en el temps i faciliten les recaigudes fins i tot molt de temps despr\u00e9s d\u2019haver deixat de consumir-la.<\/p><p><strong>Qu\u00e8 oferim?<\/strong><\/p><ul><li><strong>Primera entrevista d\u2019orientaci\u00f3 i informaci\u00f3.\u00a0<\/strong>Elaboraci\u00f3 de la hist\u00f2ria cl\u00ednica per determinar l\u2019historial de consum, els antecedents personals i familiars, la situaci\u00f3 sociofamiliar, la patologia org\u00e0nica, etc. En aquesta entrevista es determina la necessitat o no de fer una desintoxicaci\u00f3 pr\u00e8via a l\u2019inici del tractament psicoterap\u00e8utic i si aquest ha de ser ambulatori o residencial.<\/li><li><strong>Si el tractament \u00e9s residencial:\u00a0<\/strong>derivaci\u00f3 a un dels nostres dos centres terap\u00e8utics:\u00a0<a title=\"Centre Terap\u00e8utic Intensiu \u00abRiera Major\u00bb\" href=\"https:\/\/tratamientodelasadicciones.org\/tratamientos\/centro-terapeutico-intensivo-riera-major\/\" target=\"_blank\" rel=\"noopener\"><strong>Riera Major<\/strong><\/a>\u00a0o\u00a0<a title=\"Centre Terap\u00e8utic d\u2019Estada Mitjana \u00abCan Coll\u00bb\" href=\"https:\/\/tratamientodelasadicciones.org\/tratamientos\/centro-terapeutico-de-estancia-media-can-coll\/\" target=\"_blank\" rel=\"noopener\"><strong>Can Coll<\/strong><\/a>. En tots dos centres es pot fer la desintoxicaci\u00f3. Possibilitat tamb\u00e9 de fer el manteniment amb metadona o altres agonistes\/antagonistes opiacis.<\/li><li>\u00a0<strong>Si el tractament \u00e9s ambulatori:<\/strong><ul><li><strong>Atenci\u00f3 psicoterap\u00e8utica individual: sessions d\u2019orientaci\u00f3 i tractament amb un psic\u00f2leg especialista en drogodepend\u00e8ncies.<\/strong><\/li><li><strong>Atenci\u00f3 psiqui\u00e0trica<\/strong>: orientaci\u00f3 i valoraci\u00f3 de si cal introduir medicaci\u00f3 psicofarmacol\u00f2gica que atenu\u00ef el desig de consumir alcohol (craving) i la p\u00e8rdua de control un cop es comen\u00e7a a consumir (priming).<\/li><li><strong>Control d\u2019anal\u00edtiques d\u2019orina<\/strong><\/li><li><strong>Ter\u00e0pies
